This role supports the delivery of high-quality, evidence-based geriatric care to improve resident outcomes, reduce avoidable emergency department visits and hospital admissions, and support timely transitions back to LTC.The NLOT Nurse delivers mobile clinical assessment and intervention for residents experiencing acute or episodic changes, including functional decline. Working collaboratively with Physicians, Nurse Practitioners, Nurses, geriatric teams, Ontario Health at Home, residents, families, and community partners, this role supports complex geriatric care, including frailty, dementia, chronic disease, and palliative/end-of-life care.The role focuses on capacity building within LTCHs through education, coaching, and implementation of clinical best practices, while facilitating access to appropriate community and ambulatory services to ensure coordinated, resident-centred care.Primary ResponsibilitiesCollaborate to enhance the return of residents to LTC homes following unavoidable ED visits, providing additional support and mentorship to LTC staff as needed.Provide comprehensive assessment, intervention, monitoring, and follow-up care for acute and chronic illnesses.Deliver best practice education and support to caregivers, families, and LTC home staff, as well as other stakeholders upon request.Coordinate care and promote excellence in patient-centered nursing through expert practice, education, consultation, and research.As a member of the inter-professional team, the NLOT RN will practice in accordance with relevant regulatory acts and standards, including the Regulated Health Professions Act, the Gerontological Nurses Association (GNA) Standards, the Long-Term Care Homes Act, and the Practice Standards of the College of Nurses of Ontario. The NLOT RN will implement and maintain effective programs to reduce acute care admissions from the LTC sector, emphasizing wellness, disease prevention, health promotion, staff mentoring, outreach, advocacy, and community development.
