We are open to UCP's with a variety of backgrounds and training expertise.Shift Details:Employment Type: Full timePay Range: $20 - $23/ hourShift timing: 8 hour - 12 hour rotational shifts between 6AM and 11PMSchedule:10 shifts biweekly, every other weekend requiredThe CareKW Difference:Competitive starting wagesContinuing education and training programsMentorship opportunitiesHealth care Spending AccountGroup Retirement Savings Program'Responsibilities:Complete regular caregiver skill evaluations, address concerns as appropriate in collaboration with nursing team.Address client or caregiver queries or concerns related to the provision of care (in person or by telephone)Perform client intake assessments prior to the start of services and complete client update assessments as needed throughout the duration of service; communicate focus of intervention to client and familyCreate and maintain client care plans based on client's identified care needs; communicate changes in client's health status or discrepancies in assigned care tasks to community care coordinatorBe adaptable in covering open shifts and display willingness to adjust work schedule to ensure coverage in the communityWork with clients to establish meaningful goals to increase their function at homeWork as a dynamic part of a multidisciplinary teamAssist with policy creation as neededBe responsive to emails and calls as neededProvide client-centered, family-centered careMust be available for rotating shifts.Requirements:Previous experience in providing person carePreferred post-secondary education in related care fields of PSW/HCA/ DSW certificationHold a valid G drivers license and willing to commute between Brantford, St Catharines and HamiltonWilling to commute to Kitchener once per weekValid work/study permit (if applicable)Current CPR/First Aid CertificationClean Vulnerable Sector Screening (within 3 months of hire)Up-to-date Immunization Records (MMR, TDAP, Varicella, Hep B)Gentle Persuasive Approach (GPA) considered an assetNon-Violence Crisis Intervention (NVCI) considered an assetDocumentation skillsAbility to follow client care planStrong communication skillsStrong problem-solving skillsExcellent patient service skillsCareKW is committed to a culture of equity, inclusion, diversity and anti-racism.
