Our spectrum of services has grown from the delivery of Chinese meals‐on‐wheels service to a full range of community support services, covering the whole Greater Toronto Area.With a staff of more than 550 members (including full‐time and part‐time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home‐bound" and frail.We are currently seeking a Community Ambassador to fill a vacant position at our Community Health Team, with a primary focus on supporting our Black Health Initiative. This role plays a key part in advancing health equity by working closely with Black communities to improve access to preventive care, health education, screening, and system navigation.Carefirst strongly encourages applications from individuals who identify as Black, African, Caribbean, or Afro‐descendant, as well as candidates from other equity‐deserving groups. Lived experience within Black communities is considered a strong asset for this role.About the Community Health TeamThe Carefirst Community Health Team supports population health by bringing preventive care, health education, screening, and system navigation directly into the community, with a focus on equity and early intervention. including:Preventive Care ClinicCancer Prevention ProgramBlack Health InitiativeThese programs aim to improve equitable access to care, promote wellness, and strengthen connections within diverse communities.Terms: Full Time PermanentLocation: 300 Silver Star Blvd., Scarborough and community locations in the GTAPay range: $42,000 – $50,000 annuallyResponsibilitiesWorking closely with an interprofessional team, the Community Ambassador will play an integral role of: engaging in culturally responsive engagement and outreach to individual community members and provide accurate and relevant information/education about the importance of preventive health care, cancer screening, vaccination, chronic disease prevention and management, mental health promotions, access to primary care, and other local population health challengesConduct culturally and linguistically responsive educational presentations individually and within groups in the communitiesEngage community members in preventive care conversations and health action planningConnect community members to the right services and programs that support the determinants of health (i.E. service navigation to relevant local community resources). E.G. help facilitate primary care, cancer screening referrals.Conduct grassroot community outreach activities, i.E.
