As a Case Manager at Cota, you will provide critical case management services by evaluating mental health needs and coordinating resources for clients. Key Responsibilities:
• Screen applicants and confirm program eligibility
• Build supportive relationships with assigned clients
• Coordinate access to mental health and social services
• Conduct comprehensive assessments of client needs
• Maintain accurate documentation of client interactions

Requirements:
• Post-secondary education and 1-3 years of relevant experience
• Familiarity with Toronto’s community resources
• Strong analytical and support skills
• Excellent communication and teamwork abilities
• Access to a vehicle necessary
