The Support Worker is responsible for providing non-personal care support services to seniors and community members living independently within the community. The Support Worker assists individuals in maintaining their independence, quality of life, safety, and connection to community resources through outreach, companionship, transportation support, wellness checks, and assistance with activities of daily living that do not involve direct personal care.

The Support Worker works collaboratively with community partners, families, healthcare providers,



and the interdisciplinary team to support client well-being and promote aging in place. This position is community-based and may involve travel throughout the service area. The Support Worker reports directly to the Director of Finance and Health Services.

Required Skills

Experience working with seniors, individuals with disabilities, or vulnerable populations considered an asset.

Solid interpersonal, communication, and organizational skills.

Ability to work independently and as part of a team.

Valid driver’s license and reliable transportation may be required
