Our team-based model of care supports innovation, collaboration, and patient-centered service delivery.This is an exciting opportunity to make a meaningful impact in a supportive rural healthcare setting while enjoying an exceptional work-life balance in Northern Ontario.About the RoleThe Social Worker will provide mental health assessment, treatment, and short-term counselling services to patients with a wide range of presenting concerns. Working both independently and collaboratively within an interdisciplinary primary care setting, the successful candidate will:Maintain a clinical caseload providing evidence-based mental health careTriage physician and interdisciplinary referrals to the FHT Mental Health ProgramSupport patients in accessing appropriate community and social servicesCollaborate closely with physicians, nurse practitioners,



and interdisciplinary health professionals to develop comprehensive care plansParticipate in community partnerships and initiatives aimed at improving mental health supports within Elliot Lake and the surrounding areaContribute to a positive, patient-focused, team-oriented clinical environmentQualificationsThe ideal candidate will possess:Master's Degree in Social Work (MSW)Registration and membership in good standing with the Ontario College of Social Workers and Social Service WorkersValid professional liability insuranceMinimum 2–3 years of clinical experience, preferably in primary care or community mental healthPreferred Skills & ExperienceStrong clinical assessment, treatment, and case management skillsExperience supporting mental health concerns commonly seen in primary care settingsKnowledge of CBT, DBT, and ACT therapeutic approachesExperience facilitating groups and psychoeducational programmingStrong systems navigation skills and familiarity with community resourcesKnowledge of the Mental Health Act and related legislationExperience working with older adults/geriatric populations is considered an assetFamiliarity with electronic medical records (EMR)Knowledge of privacy legislation and PHIPA requirementsBilingualism (English/French) is considered an assetSalary & BenefitsProfessional Development FundingFour Weeks Vacation to Start, Paid Personal and Sick TimeStarting salary will commensurate with education and experience, max $76,150How to ApplyQualified applicants are invited to submit a cover letter and resume via email to:[email protected] thank all applicants for their interest; however, only those selected for an interview will be contacted.
