As a vital part of the weCHC team, you will deliver client assessments, collaborate on treatment plans, and lead educational initiatives for patients dealing with diabetes. Engage in community networking and quality improvement activities while working under the College of Nurses of Ontario standards.

Key Responsibilities:
• Conduct thorough assessments and develop treatment strategies
• Collaborate with interdisciplinary healthcare teams
• Monitor patient progress and participation in care plans




• Facilitate health education and promotion programs
• Document care and participate in reviews and discussions

Requirements:
• A minimum of 3-5 years of nursing experience
• In progress or completion of Certified Diabetes Educator designation
• Robust understanding of diabetes types and community services
• Excellent organizational and communication abilities
• Class “G” Driver’s License for travel around Windsor-Essex
