**About the Role**
The Clinical Lead, Access & Flow is a critical member of the inpatient Access & Flow team.

The inpatient Access & Flow team works collaboratively across the inpatient program to provide leadership and direction related to access and flow throughout the care continuum.

The team's objective is to deliver an exceptional client/family experience while managing and adapting in response to broader health system pressures.

Ultimately, goal of this team is to work collaboratively with the client/family, internal and external partners to achieve a well-coordinated, comprehensive, seamless client experience while optimizing system resources.

The Clinical Lead, Access & Flow provides system navigation for clients and families as they move from acute care services to rehabilitation services and then as they integrate into the community.

Clients may range from birth to 18 years of age with a variety of medically complex conditions including brain injury, spinal cord injury, and orthopaedic surgery.

Access & Flow utilizes their knowledge and understanding of health care system and community resources to enhance operational effectiveness of the inpatient units and day program while supporting a seamless care experience for clients and families.
**Key Responsibilities**
Clinical Practice: Reviewing and supporting the assessment of all incoming referrals, including the rehabilitation goals of the client and family in partnership with the multi-disciplinary team
Liaising and establishing contact with members of the multi-disciplinary team and appropriate care providers to obtain appropriate information relating to the client's care
Managing a caseload, preparing complete and accurate information, tracking and trending data, and prioritizing workload to promote access, flow across the paediatric health system
Interprofessional Collaborations: Collaborates with interprofessional team members, clients and families as well as community healthcare providers and agencies to achieve seamless transitions
Participates in quality improvement initiatives and committees working towards optimal delivery of client care
Builds team-oriented relationships with key partners (e.g.

Sick

Kids, OH@H, Safehaven, Children's Treatment Centres and other referring partners)
Leadership: Guides, initiates and provides leadership to support operational effectiveness and client flow (e.g. LOS, wait times)
Examines current processes, identifies opportunities for change and provides recommendations for improvements
Research and Education: Uses evidence in practice, develops and uses clinical practice guidelines, pathways and protocols in transition planning and service delivery
Functions as a content expert and resource person in the field of access and flow, and takes advantage of informal and formal learning opportunities to educate the interprofessional team
Educates clients, families, members of the health care team, referring facilities and other community providers
**Qualifications**
Education
A Baccalaureate Degree or equivalency in health-related discipline, Master's Degree preferred
Requires active registration as a Regulated Health Care Professional in Ontario
Experience
Minimum 3 years pediatric experience; experience working with families required




Experience working with children with disabilities and/or complex care needs and medical technologies preferred
Skills
Affinity for working in a fast-paced environment and confidence in thinking critically and making autonomous decisions
Strong knowledge-base related to current community resources
Experience championing system improvements by managing a team through project life cycles
Ability to prioritize workload, anticipate challenges, delegate, and effectively escalate issues when required
Knowledge and experience using utilization management principles and length of stay management strategies.

Excellent organizational, interpersonal and communication skills.

Excellent time management skills with ability to multi-task.
