Griswold Home Care is a locally owned, private-pay home care agency serving seniors and families across the SE Houston area. We help clients stay safe, comfortable, and independent at home, and much of our business comes from word of mouth. That happens because we take care of our clients and our caregivers. We're looking for an organized, people-focused Care Coordinator to help us keep that standard high.

Position Summary The Care Coordinator is the day-to-day link between our clients, their families, and our caregivers. You'll handle scheduling and staffing, assist with client intakes, conduct client reassessments and home visits, support caregiver recruiting and performance reviews, and make sure every client gets consistent, high-quality care. This is a hands-on role for someone who enjoys solving problems and building relationships.

Key Responsibilities

Client Care & Visits

- Assist with new client intakes and in-home assessments, working alongside our Client Intake & Community Liaison to ensure a smooth start of service.
- Conduct client reassessments when care needs change, after hospital or rehab stays, and at routine intervals, then update care plans to match.
- Develop and update client care plans with input from clients, families, and leadership.
- Perform routine client visits and supervisory check-ins to monitor satisfaction and care quality.
- Serve as a main point of contact for clients and families, responding promptly to questions, concerns, and schedule changes.

Scheduling & Staffing

- Build and maintain client schedules, matching caregivers to clients based on skills, personality, and availability.
- Fill open shifts and call-offs quickly to prevent missed visits.
- Monitor visit verification, timekeeping, and caregiver attendance.
- Take after-hours on-call duty at least once a month, and more often as needed, handling call-offs, scheduling issues, and client or caregiver concerns.

Caregiver Recruiting & Support





- Assist with caregiver recruiting, including posting ads, screening applicants, scheduling and conducting interviews, and checking references.
- Support new-hire onboarding and orientation.
- Conduct annual caregiver performance reviews and assist with ongoing coaching and field observations.
- Build positive relationships with caregivers to support retention and job satisfaction.

Administrative & Compliance

- Maintain accurate client and caregiver records in our scheduling software.
- Document incidents, complaints, and care plan changes, and escalate issues to leadership as needed.
- Help ensure the agency follows company policies and Texas HHSC regulations for personal assistance services.
- Perform other duties as assigned.

Qualifications

- 1–2+ years of experience in home care, home health, senior care, or a similar healthcare setting (care coordination or scheduling experience strongly preferred).
- CNA, CMA, LVN, or social work background a plus but not required.
- Able to work after-hours on-call at least once a month, and more often as required.
- Excellent communication skills, with the ability to stay calm and kind under pressure.
- Strong organizational skills and the ability to juggle multiple priorities.
- Comfortable with computers, scheduling software, and Microsoft Office or Google Workspace.
- Valid Texas driver's license, reliable transportation, and auto insurance for client visits.
- Must pass a background check and drug screen.
- Bilingual (English/Spanish) a plus.
- Ability to work weekends and after hours when necessary.

Compensation & Benefits

- $50,000–$60,000 per year, based on experience

Schedule Monday–Friday, [8:30 AM–5:00 PM], plus after-hours on-call at least once a month (more as required) and occasional evening or weekend client needs.
