JOB SUMMARY:

Within the context of a client and family centred model of care, and in accordance with the Mission, Vision, Values and strategic directions of Providence Health Care, the person promotes a secure, respectful, and civil working environment for patients, residents, families, visitors, and staff.

Reporting to the Patient Care Manager, the Clinical Nurse Leader/Triage Coordinator, coordinates the day-to-day access and flow operations of a designated nursing units across the Heart Centre and Critical Care. This position provides a program/unit perspective on patient placement, bed utilization and staffing. Assesses situations, prioritizes, and expedites critical issues, and facilitates the use of staffing and other resources to ensure optimal patient care.

Acts as a clinical resource to nursing staff and interdisciplinary team members in the provision of direct care to patients/residents. Assists the Patient Care Manager in assessing flow and access needs of the designated program and needs/interests of staff. Assists in the promotion of research and quality improvement initiatives within the program team.

QUALIFICATIONS:

Education, Training and Experience

Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a practicing RN registrant, qualifications required of staff nurse in clinical area, plus three years of recent related experience in clinical specialty, or an equivalent combination of education, training, and experience.

Practices in accordance with British Columbia College of Nurses and Midwives Standards.

- Coordinates and oversees patient placement activities related to bed utilization and bed space allotments for the Heart Centre and Critical Care, by performing duties such as assessing and monitoring waitlists, triaging patients based on condition and available resources, prioritizing, and expediting critical issues requiring immediate action. Negotiates changes with staff, facilitates the transfer of resources and evaluates potential patient and resource implications to ensure optimal flow into and across units.



Problem-solves and troubleshoots patient placement issues.
- Acts as a resource for the program and units on issues related to patient flow, bed utilization and staffing requirements by performing duties such as assisting Clinical Site Coordinators in the allocation of beds and interfacing between program units the broader organization and the provincial Patient Transfer Network (PTN). Monitors patienVresident acuity and flow of patients/residents across the Heart Centre and Critical Care. Makes decisions to ensure that resources are appropriate for patient acuity. Makes adjustments in response to changes.
- Acts as a clinical resource to nursing staff, students, and other members of the interdisciplinary team, by problem solving, and troubleshooting concerns related to patient placement and available resources.

Advises Patient Care

Manager and /or resource person of issues/concerns.

- Participates in rounds/clinics as needed and collaborates with the interdisciplinary team and physicians to assess patient/resident condition and needs and plans and prioritizes bed placement accordingly.
- Collaborates with interdisciplinary team to determine need for additional consultation with other disciplines or departments.
- Acts as the patienVresident and family advocate by communicating concerns to other members of the patient/resident care team and by supporting culturally safe and responsive care in alignment with PHC commitment to Truth and Reconciliation. Intervenes as required.
- Performs direct nursing care as required by assuming responsibility for a patient/resident assignment.
- Provides input into the development, evaluation, and delivery of orientation and resource materials, in conjunction with other members of the health care team and participates in the orientation of other clinical leaders and new employees as required. Makes recommendations and provides leadership regarding changes or new initiatives.
- Presents in-service and educational sessions for staff and physicians, as required.
- Reviews and participates in the development of policies, procedures, and reference materials, and provides feedback and input to the Patient Care Manager or designate for changes and modifications. Updates as approved.
- Assists the Patient Care Manager and other resource nurses in promoting further education, by mentoring staff regarding educational opportunities,



and assisting with interpersonal conflict resolution and the development of clinical and leadership skill.
- Provides input to the Patient Care Manager regarding Performance Planning and Coaching of unit staff. Acts as clinical resource to unit staff in the Performance Planning and Coaching process.
- Assists the nurses and other members of the interdisciplinary team in the identification of researchable clinical patient/resident care management problems. Participates in research initiatives and projects related to patient/resident care and/or access and flow as requested, disseminates findings, and assists in applying and implementing research findings into practice.
- Collaborates with interdisciplinary care team members regarding quality improvement initiatives, by making recommendations for QI projects, implementing processes and procedures upon authorization, and evaluating results. makes adjustments, as required, upon authorization of Patient Care Manager.
- Performs other related duties as assigned.
