Position Summary: The Clinical Pharmacist provides direct patient care in a community pharmacy, using the full scope of practice in Alberta to optimize medication therapy, improve health outcomes, and support safe, effective drug use. The pharmacist delivers services in alignment with the Alberta Health Pharmacy Services Framework and the standards of the Alberta College of Pharmacy (ACP).

Key Responsibilities:

1. Patient Assessment & Care

- Obtain and document medical and medication histories; assess indication, effectiveness, safety, and adherence.
- Identify and resolve drug therapy problems, including interactions, duplications, therapeutic gaps, and inappropriate therapy.
- Provide disease-state management services as needed
- Conduct comprehensive annual care plans and medication management assessment as outlined in the Alberta Health Pharmacy Services Framework.
- Provide all professional services within the Alberta Health Pharmacy Services Framework, including assessments, prescription renewals, adaptations, prescribing services where authorized, follow-ups, and other billable pharmacy services.
- Document assessments, care plans, and follow-ups in accordance with ACP standards and payer requirements.

2. Prescribing & Expanded Scope (as authorized)

- Prescribe medications under Additional Prescribing Authorization (APA), where obtained and appropriate.
- Adapt prescriptions (e.g., change dose, regimen, formulation, or therapeutic substitution where permitted).
- Extend, renew, or manage ongoing chronic medication therapy.
- Order, interpret, and use laboratory tests to support medication management.
- Administer drugs by injection and other authorized routes (e.g., vaccines, biologics, long-acting injectables).

3. Dispensing & Technical Oversight





- Review and verify prescriptions for clinical appropriateness, accuracy, and legal requirements.
- Perform final checks on all dispensed prescriptions.
- Oversee and support pharmacy technicians and assistants, delegating tasks appropriately.
- Ensure safe, accurate preparation, labeling, and documentation of prescriptions, including high-alert and controlled medications.

4. Patient Education & Counseling

- Provide patient-centered counseling on new and existing prescriptions (indication, dosing, administration, side effects, monitoring, adherence).
- Educate patients on lifestyle modifications, self-management strategies, and non-pharmacologic options.
- Guide protected and appropriate use of non-prescription, natural health, and minor ailment therapies.

5. Clinical Programs & Chronic Disease Management

- Develop and deliver clinical services (e.g., medication reviews, vaccination clinics, smoking cessation, opioid stewardship, cardiovascular/diabetes clinics).
- Create individualized care plans with measurable goals, monitoring parameters, and follow-up timelines.
- Track and evaluate clinical outcomes and contribute to continuous improvement of clinical programs.

6. Collaboration & Communication

- Collaborate with physicians, nurse practitioners, nurses, and other healthcare professionals to coordinate patient care.
- Communicate care plans, recommendations, and significant drug-related issues to prescribers and other team members.




- Participate in interdisciplinary initiatives (e.g., Primary Care Networks, attached/embedded clinic models, virtual care where applicable).

7. Regulatory Compliance, Safety & Quality

- Comply with ACP standards of practice, Code of Ethics, and relevant federal/provincial legislation (e.g., Food and Drugs Act, CDSA, Narcotic Control Regulations).
- Maintain patient confidentiality and secure, accurate records.
- Ensure compliant handling, storage, inventory control, and destruction of controlled substances and all medications.
- Participate in incident reporting, quality assurance, and practice-improvement activities.

8. Professional Development

- Maintain active registration and good standing with the Alberta College of Pharmacy.
- Complete ACP Continuing Competence Program requirements and employer-mandated training.
- Stay current with clinical guidelines, new therapies, and evolving Alberta Health/ACP policies and frameworks.

Qualifications

- Bachelor of Science in Pharmacy (BScPharm) or Doctor of Pharmacy (PharmD).
- Licensed (or eligible for licensure) as a pharmacist with the Alberta College of Pharmacy.
- Authorization to administer injections (or ability to obtain within a specified timeframe).
- Professional liability insurance meeting ACP requirements.
- Additional Prescribing Authorization (APA) preferred or commitment to obtain within a defined period.
- Community/retail pharmacy experience and demonstrated interest in clinical services and chronic disease management preferred.
- Strong communication, documentation, clinical problem-solving, and time-management skills.

Pay: $50.00-$60.00 per hour Expected hours: 40.0 per week
