- Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
- Implementsplan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural,ethnicand religious/spiritual needs.




- Provides direct patient/resident care andreportscondition and changes to RN or LPN.
- Demonstrates time management and prioritization of tasks.
- Document results of patient/resident care within established policies,proceduresand scope of practice.
- Nutritional care - Effective communication with residents and family - Completes all other duties as assigned. ___ Qualifications: Required:
- Must be in good standing on the State Nursing Assistant registry orappropriate entity - Ability to meet physical and job-related requirements - Must be English literate andpossessexcellent writtenandverbal communication skills Preferred:
