Community Health Representative (CHR)

Reports To

Health Director or designate; works collaboratively with the Head Nurse, Community Health Nurses and the broader Tl’azt’en Nation Health Department.

Position Summary The Community Health Representative (CHR) supports Tl’azt’en Nation members in accessing health services, health information, prevention programs and community wellness resources.

The CHR acts as an important connection between community members, families, the Tl’azt’en Nation Health Department and external healthcare providers. The position focuses on health promotion, prevention, community outreach, client advocacy, health-system navigation and culturally safe communication.

Working closely with the nursing team and other Health Department programs, the CHR helps identify community health needs, provides education and outreach, assists clients with accessing services and supports community-based health initiatives.

Key Responsibilities

Community Outreach and Client Support

- Build respectful and trusting relationships with community members, Elders, youth and families.
- Provide information about available Health Department programs and services.
- Assist community members in connecting with appropriate health, wellness and social services.
- Provide health-system navigation and support clients in understanding referrals, appointments and healthcare processes.
- Follow up with clients when requested by the nursing or Health Department team.
- Identify barriers preventing community members from accessing care and bring those concerns forward to the appropriate program or manager.
- Advocate for culturally safe, respectful and accessible services for Tl’azt’en Nation members.

Health Promotion and Prevention

- Assist in planning and delivering community health-promotion and disease-prevention programs.
- Provide education regarding healthy lifestyles, chronic-disease prevention, communicable disease, maternal and child health, sexual health, substance-use prevention and other identified community priorities.
- Support health fairs, screening events, vaccination clinics and wellness initiatives.
- Distribute approved health-education materials and community health notices.
- Encourage participation in preventative screening and routine healthcare.
- Assist nursing staff with community outreach and awareness activities.

Public and Community Health Support

- Work alongside Community Health Nurses and other clinical staff in delivering community health programs.




- Support communicable-disease prevention, education and community outreach under the direction of regulated health professionals.
- Assist with community-health surveillance by identifying and reporting emerging community concerns through established processes.
- Support public-health initiatives relating to environmental health, emergency response, infection prevention and community wellness where appropriate.
- Participate in emergency-preparedness and community-response activities as assigned.

Client Advocacy and Health Navigation

- Help community members understand available health programs, benefits and referral pathways.
- Assist clients in identifying appropriate local, regional, FNHA and provincial healthcare resources.
- Support communication between clients and healthcare providers when appropriate and with consent.
- Assist community members experiencing barriers related to transportation, appointments, documentation or accessing services by connecting them with the appropriate Health Department program.
- Promote client independence and informed participation in healthcare decisions.

Community Engagement

- Gather community feedback regarding health needs, service gaps and program priorities.
- Assist with community needs assessments and health-program planning.
- Promote Health Department programs through community events, outreach and approved communications.
- Support Elders, youth, families and other community groups in accessing preventative health information.
- Participate in community events and culturally relevant activities as a representative of the Health Department.

Documentation and Administration

- Maintain accurate records of outreach activities, client contacts and program participation.
- Assist with program reporting and data collection.
- Maintain strict confidentiality of all personal and health information.
- Participate in staff meetings, training and professional-development opportunities.
- Prepare basic reports, statistics and program updates as required.
- Follow Tl’azt’en Nation policies, Health Department procedures and applicable privacy requirements.

Qualifications A combination of education,



community knowledge and relevant experience will be considered.

Preferred qualifications include

- Certificate, diploma or coursework in Community Health, Indigenous Health, Health Care, Social Services, Community Support or a related field.
- Previous experience in community health, health promotion, outreach, Indigenous services or a related position.
- Knowledge of Tl’azt’en Nation community, culture and local resources is a strong asset.
- Experience working within a First Nations community is considered an asset.
- Current First Aid/CPR certification or willingness to obtain.
- Valid BC Driver’s Licence and reliable transportation.
- Successful Criminal Record Check and Vulnerable Sector Check.

- Understanding of cultural safety, trauma-informed practice and Indigenous approaches to health and wellness.
- Solid awareness of confidentiality and professional boundaries.
- Ability to work both independently and collaboratively.
- Comfortable conducting community outreach and home visits.
- Ability to organize activities, maintain records and meet reporting requirements.
- Ability to recognize concerns that require referral to a nurse, physician, mental-health professional or other regulated professional.
- Strong advocacy and health-system navigation skills.
- Ability to use email, Microsoft Office/Teams and basic electronic documentation systems.

Scope of Role The Community Health Representative is not a regulated nursing position and does not independently diagnose, assess or provide medical treatment. Clinical concerns identified by the CHR are referred to the appropriate nurse, physician, Nurse Practitioner, mental-health clinician or other regulated healthcare provider.

Working Conditions

- Work takes place within Tl’azt’en Nation communities, the Health Centre, community facilities and client homes.
- Regular community outreach and local travel are required.
- Some evening or community-event participation may occasionally be required.
- The role requires flexibility in responding to community and departmental priorities.
