Concurrent Disorder Specialist – Mental Health & Justice Case Management - Full-Time Contract (6 Months)

The Role and Opportunity

CMHA Toronto is seeking a compassionate, self-driven, and collaborative Concurrent Disorder Specialist to join our Mental Health & Justice Case Management (MH&J; CM) Team. This role is ideal for someone who takes initiative, builds rapport with ease, and brings warmth and professionalism to every interaction.

You’ll support individuals living with mental health and addiction challenges who also have involvement with the criminal justice system. Our work is grounded in Housing First, harm reduction, and trauma-informed principles, with a strong focus on community integration and recovery.

Salary: $32.30 - $35.39 + 4% in lieu of vacation

Hours: 35 per week

Location: 700 Lawrence Ave. W + community-based work

Responsibilities

What You’ll Do

- Conduct outreach and assessments
- Provide program and agency orientation
- Deliver compassionate, client centered engagement and interventions
- Support skill development and collaborative goal planning
- Offer resources, information, and advocacy
- Build trusting, respectful, and easygoing working relationships
- Monitor progress, evaluate needs, and provide follow up
- Facilitate individual and group addictions counselling
- Support agency-wide concurrent disorder initiatives
- Consult with staff on addictions related matters

Skills You Bring You’re someone who leads with empathy, takes initiative,



and thrives both independently and in a team. You communicate with clarity, listen deeply, and bring a grounded presence to challenging situations.
- Bachelor's degree in social work, Social Science, or equivalent
- Addictions Diploma/Certificate or equivalent experience
- 3+ years in community mental health and addictions
- Strong understanding of Recovery principles & Strengths Based practice
- Knowledge of Trauma-informed Care
- Experience with concurrent disorders, homelessness, and the justice system
- Ability to respond to crises with compassion and sound judgment
- Excellent communication, listening, and interpersonal skills
- Strong organizational skills and self motivation
- Commitment to antiracism and nonhomophobic practice
- Second language an asset
- Occasional evening availability
- Access to a car + valid driver’s license
