The work schedule is Monday to Friday from 9 AM to 4:30 PM. In this role, you will oversee a caseload of nine clients while working collaboratively with professionals in the field.

Your mission will be to deliver effective rehabilitation strategies and coordinate long-term support services that enhance quality of life. Previous experience in health or social services is essential for achieving success in this position. Key Responsibilities:

- Implement client-focused rehabilitation programs
- Manage nine active client cases weekly
- Work on multidisciplinary team reports
- Support families and staff as a resource
- Conduct daily travel in York Region for client visits Requirements:
- 2-5 years of social services experience
- Concrete knowledge of ABI effects
- Background in community services for disabilities
- Strong time management and problem-solving skills
