Reports To

Health Director or designate; works operationally under the direction of the Head Nurse/Home & Community Care Lead.

Position Summary The Home Care Worker provides respectful, culturally safe, client-centred support to Tl’azt’en Nation members who require assistance to remain safely and independently in their homes.

Working under the direction of the Health Department and in collaboration with the Head Nurse/Home & Community Care team, the Home Care Worker assists Elders, adults with disabilities, individuals recovering from illness or hospitalization, and other eligible community members with activities of daily living, household support, wellness monitoring, and connection to health services.

This position requires compassion, reliability, confidentiality, strong boundaries, and the ability to work independently in community members’ homes.

Key Responsibilities

Client Care and Daily Living Support

- Provide personal care and assistance with activities of daily living within the employee’s level of training and assigned care plan.
- Assist clients with mobility, transfers, positioning, dressing, grooming, bathing and other approved personal-care activities.
- Provide meal preparation and basic nutrition support.
- Assist with light housekeeping, laundry and maintaining a safe home workplace.
- Support clients with approved errands, appointments or essential activities where assigned.
- Encourage independence and participation in daily activities whenever possible.
- Provide companionship, social interaction and wellness support, particularly for Elders and clients experiencing isolation.

Home and Community Care

- Follow individualized client care plans developed by nursing or other authorized health professionals.
- Observe clients for changes in physical,



emotional or functional condition and report concerns promptly to the supervising nurse or designated Health Department lead.
- Support discharge and recovery plans following hospitalization where directed.
- Assist with fall-prevention strategies and maintaining a safe home environment.
- Identify potential safety concerns in clients’ homes and report them through the appropriate Health Department process.
- Maintain professional boundaries while providing compassionate and culturally appropriate support.

Team Collaboration

- Work closely with Community Health Nurses, the Head Nurse, LPNs and other members of the Health Department.
- Participate in case planning, team meetings and client-care discussions as required.
- Communicate significant client concerns, missed visits or changes in condition promptly.
- Coordinate with other Tl’azt’en Nation programs where appropriate to support holistic client needs.
- Participate in community wellness initiatives, Elders programming and health-promotion activities when requested.

Documentation and Confidentiality

- Complete daily service records, client notes, mileage records, incident reports and other required documentation accurately and on time.
- Maintain strict confidentiality of client, family and community information.
- Follow Tl’azt’en Nation privacy, confidentiality and records-management requirements.
- Immediately report suspected abuse, neglect, unsafe conditions or other serious concerns according to policy and applicable legislation.

Qualifications

- Health Care Assistant, Community Health Worker, Personal Support Worker or equivalent training is preferred.
- Relevant experience supporting Elders,



individuals with disabilities or clients receiving home and community care may be considered.
- Previous experience working in a First Nations community or Indigenous health setting is an asset.
- Current First Aid/CPR certification or willingness to obtain.
- Valid BC Driver’s Licence and reliable transportation are strongly preferred.
- Successful Criminal Record Check and Vulnerable Sector Check required.
- Food Safe certification is an asset.

Working Conditions

- Work primarily takes place in community members’ homes and throughout Tl’azt’en Nation communities.
- Local travel is required.
- The position may involve standing, bending, lifting, assisting with transfers and other physical activities associated with personal and home care.
- Exposure to varying home environments may occur.
- Hours may vary depending on client and program needs.
