Reports To

Health Director or designate; works clinically in collaboration with the Head Nurse and nursing team.

Position Summary The Licensed Practical Nurse provides community-based nursing services to Tl’azt’en Nation members across the lifespan while supporting the delivery and continued development of Home & Community Care and community health programming.

Working within the LPN scope of practice and in collaboration with the Head Nurse, Community Health Nurses, physicians, Nurse Practitioners, Home Care staff and other members of the interdisciplinary team, the LPN provides clinical care, health monitoring, chronic-disease support, medication-related services, wound care, client education and care coordination.

The position requires strong clinical judgment, professionalism, cultural safety, flexibility and the ability to work independently in a community setting.

Key Responsibilities

Clinical Nursing Services

- Provide nursing care in accordance with BCCNM standards, organizational policies and individual scope of practice.
- Conduct client assessments and monitor changes in health status.
- Measure and monitor vital signs and other clinical indicators as appropriate.
- Provide wound assessment, wound care and dressing changes within scope.
- Support medication management, medication reconciliation and medication administration where authorized.
- Provide injections, treatments and other nursing interventions within scope and organizational policy.
- Assist with chronic-disease management including diabetes, cardiovascular conditions and other ongoing health concerns.
- Provide post-hospitalization follow-up and support continuity of care.
- Support palliative and end-of-life care in collaboration with the broader care team where appropriate.
- Recognize clinical deterioration or urgent health concerns and escalate appropriately.

Home & Community Care
- Conduct home visits and provide nursing services to clients who may have difficulty accessing clinic-based care.
- Participate in the development, implementation and review of individualized care plans.




- Work with Home Care Support Workers to ensure client services are delivered safely and consistently.
- Provide direction and education to unregulated care providers within appropriate scope and organizational structure.
- Support Elders and individuals living with chronic illness, disability or reduced mobility to remain safely in their homes.
- Identify home safety concerns and coordinate appropriate referrals or interventions.

Community Health
- Participate in community health clinics, screening initiatives and prevention programs.
- Provide health education to individuals, families and groups.
- Support immunization and public-health programming where qualifications, delegation and program requirements permit.
- Participate in communicable-disease prevention and response activities as assigned.
- Assist with wellness events, health promotion initiatives and community outreach.
- Promote preventative health, early intervention and culturally appropriate health education.

Care Coordination and Referrals
- Collaborate with Community Health Nurses, physicians, Nurse Practitioners, pharmacists, Northern Health, FNHA and other service providers.
- Coordinate referrals and follow-up for clients requiring additional assessment or services.
- Assist clients and families in understanding care recommendations and navigating health services.
- Support continuity of care between hospital, primary care, community services and the client’s home.

Documentation and Quality
- Maintain timely, accurate and confidential clinical documentation.
- Complete incident reports and other required clinical records.
- Participate in quality improvement, clinical-policy development and program evaluation.
- Ensure nursing supplies and clinical equipment are appropriately maintained and report inventory requirements.




- Follow infection prevention and control procedures and occupational health and safety requirements.

Qualifications
- Current registration and good standing as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Current CPR certification.
- Valid BC Driver’s Licence and access to reliable transportation.
- Successful Criminal Record Check and Vulnerable Sector Check.
- Experience in community nursing, primary care, home care, public health or Indigenous health is preferred.
- Experience working with First Nations communities is considered a strong asset.
- Additional certifications related to immunization, wound care, chronic disease, palliative care or community nursing are assets.

Working Conditions
- Work occurs in the Health Centre, community settings and client homes.
- Regular local travel may be required.
- The position may require standing, bending, lifting and assisting clients with mobility.
- Some flexibility in hours may be required to meet operational or community-health needs.
