Role Overview

We are seeking an experienced Life Care Planner to join our multidisciplinary team.

The Life Care

Planner develops comprehensive, evidence-based Life Care Plans for individuals who have experienced catastrophic injuries or have complex, chronic healthcare needs. Through comprehensive assessment, analysis of medical and rehabilitation information, research, and collaboration with clients and healthcare providers, the Life Care Planner identifies current and future care needs and prepares objective recommendations and associated cost projections. Working with a high degree of autonomy, this role requires strong clinical judgment, analytical thinking, exceptional organizational skills, and a commitment to delivering high-quality work within established timelines and qualified standards.

Key Responsibilities

- Conduct comprehensive client assessments, interviews, and reviews of medical and rehabilitation records.
- Develop evidence-based Life Care Plans that identify current and future care needs and associated costs.
- Research treatment options, services, equipment, and community resources to support recommendations.
- Collaborate with clients, families, healthcare providers, legal professionals, and other stakeholders to gather information and inform care planning.
- Prepare clear, objective, and well-supported reports in accordance with professional standards and organizational expectations.
- Manage an independent caseload, balancing multiple priorities while meeting deadlines and budget expectations.




- Maintain accurate documentation and ensure compliance with professional, ethical, and confidentiality requirements.
- Participate in ongoing professional development, mentorship, and team collaboration to support continuous learning and service excellence.
- Contribute to business development by fostering relationships with referral sources and representing the organization professionally.
- Travel as required to conduct assessments, attend meetings, and support client needs.

Requirements and Qualifications

- Possession of a rehabilitation related designation, such as Registered Occupational Therapist, Registered Nurse, Registered Physiotherapist, Registered Kinesiologist or other applicable degree/professional designation.
- Canadian Certified Life Care Planning (CCLCP) credential preferred, or willingness to work towards obtaining the certification
- Valid Driver’s license, reliable vehicle, and valid applicable insurance.
- Satisfactory Criminal record check with vulnerable sector screening
- Computer literacy, including effective use of MS Word, Excel, and email.
- Exceptional report writing abilities.
- Familiarity with; knowledge of: in-home occupational therapy assessments, hospital discharging, home environment, worksite and community assessments, physical demands analyses, and functional ability assessments.
- Experience with or knowledge of vocational assessments and vocational rehabilitation.
- Knowledge of the Statutory Accident Benefits Schedule (SABS).
- Knowledge of treatment plans under Ontario’s SABS regulations.




- Superior oral and written communication skills, strong decision-making and negotiation skills, a keen attention to detail, ability to effectively manage constraints and competing priorities with minimal direct supervision.
- Demonstrated success at building relationships and collaborating with a network of internal and external colleagues and subject matter experts.
- Ability to be self-directed and effectively problem-solve.
- Ability to work effectively with all levels of staff and within a multi-disciplinary team.
- Ability to demonstrate a customer service philosophy.
- Ability to function in a consult model as appropriate.

Pay: $85,000.00-$95,000.00 per year
