Become a vital part of the community by joining March of Dimes Canada as a Navigator of Community Services in Whitby, ON. This full time role is centered on consumer assessments and collaboration with local agencies. Operating from 1615 Dundas Street East, Suite 305, this position runs Monday to Friday from 9 a.m. to 4:30 p.m., providing 35 hours of dedicated service weekly.

As a Navigator, you will conduct interRAI CHA assessments, renew service agreements, and manage intake processes for consumers while supporting their access to community resources. Key Responsibilities:

- Complete interRAI CHA assessments for new consumers
- Renew annual agreements for existing clients
- Oversee intake and referrals effectively
- Liaise with community and agency stakeholders
- Manage waitlists and maintain organized files Requirements:
- Post-secondary education in health/human services
- Experience with interRAI CHA assessments preferred
- Must have current CPR/First Aid certification
- Excellent communication and organizational skills
