Care Navigator - Ontario (Remote)

As a Care Navigator you will be responsible for managing the enrollment, reimbursement process, and service coordination for patients enrolled in a Patient Support Program (PSP). You will support patients, physicians, and clinics with drug coverage, coordinate submissions to private and provincial drug plans, and help ensure timely access to prescribed therapies. You will build trusted relationships with patients and healthcare stakeholders while providing ongoing support and coordinating services across the patient journey.

To achieve success in this position you will;

- Act as the primary point of contact for patient-related elements of the Patient Support Program and provide timely, compassionate support.
- Manage timely patient enrollment and provide inbound and outbound telephone support, including maintenance and adherence support.
- Collaborate with patients, insurers, physicians, pharmacies, infusion/injection service providers, and other stakeholders to coordinate comprehensive patient services.
- Investigate public and private insurance coverage and support physicians with the documentation required to maximize reimbursement coverage.
- Review patient charts, previous therapies, and relevant tests to assist prescribing physicians with public or private coverage submissions.
- Provide program and therapy education to help manage patient and physician expectations and support appropriate access to therapy.
- Collect information and conduct patient financial eligibility assessments in accordance with program guidelines.
- Review, analyze, and validate patient information for accuracy and completeness to support initiation and continuation of services.
- Follow up and advocate with relevant stakeholders until coverage decisions are received, with the goal of securing timely access to therapy.




- Maintain accurate and timely patient and clinic information within the CRM and meet established program KPIs and service levels.
- Identify obstacles to obtaining coverage and communicate relevant issues to the PSP Supervisor while contributing to a collaborative team environment.
- Maintain confidentiality of patient, client, and corporate information and follow all company policies, procedures, and applicable SOPs.

What you bring to the table…

- Post-secondary education; healthcare experience is an asset.
- Bilingualism in English and French; bilingualism is required in Quebec.
- Experience with reimbursement billing, special access, appeals processes, and patient support or consultation is an asset.
- Knowledge of private and public reimbursement structures, systems, and processes is an asset.
- Excellent verbal and written communication skills.
- Strong analytical skills, including the ability to interpret regulations and legislation.
- Advanced knowledge of the pharmaceutical distribution industry is an asset.
- Excellent customer service, problem-solving, conflict resolution, and interpersonal skills.
- Strong organizational skills with the ability to manage multiple stakeholders, priorities, and time-sensitive patient needs.
- Ability to work independently from home with a quiet, private home office space.
