Humber River Health is seeking a part time Social Worker to support patient flow and discharge planning at the Wilson Site. The role involves psychosocial assessment, crisis intervention and coordination with families and community resources.
Reporting to the Manager, Access and Flow, the position requires MSW eligibility, strong communication skills and the ability to work in a fast-paced acute care environment. Weekend rotation and weekday coverage are required.
