Integrated Home Care uses a strength-based, client-centered Case Management practice framework. Using a person-centered approach, you will develop a care plan that supports clients and families to identify needs, focus on strengths, set achievable personal health and self-care goals, and receive direct nursing care. In collaboration with clients and families, you will ensure services are provided and modified on an ongoing basis based on the client's response to interventions, while incorporating stewardship into service authorization.

You will report to the Home Care Manager and be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination, and direct care in partnership with clients and their families in their own residences or alternate care settings, as determined by assessment. Client care in a Home Care setting uses a collaborative, team-based approach while working remotely in a community environment.

There is also an expectation to function as a mobile worker. This role requires travel to the communities of Airdrie and surrounding rural areas; and includes caring for clients in their homes or in a lodge setting. Possession of a valid driver's license and access to a reliable motor vehicle in good repair for work-related purposes are requirements of this position.

You will be responsible for operating a laptop computer and mobile phone and performing manual handling activities, including lifting, lowering, carrying, pushing, and pulling as a regular part of the job. For example,



lifting approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common in this role.

Description

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.

You will provide secure, quality patient and family centered care while reflecting the shared vision and values.

PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).

Current Basic Cardiac Life

Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications

Two years of recent Home Care or acute care experience within the last five years. Advanced wound care knowledge and experience are required. Proficiency in computer use, as the role is heavily dependent on electronic documentation and technology. Knowledge, skills, ability, and willingness to work to full scope of practice. Demonstrated critical thinking skills. Ability to apply critical inquiry in the assessment, planning, implementation, and evaluation of care.

Preferred Qualifications

Bachelor's degree in Nursing.

Experience working with acute clients and/or long-term clients, with a focus on chronic disease management. Recent community nursing experience; Home Care experience preferred.

Experience providing case management services. CNA Certification in Gerontology.

Experience in Home Parenteral Therapy (HPT), wound, skin, and ostomy care, palliative support, and client teaching.
