In this position within the Family Health Team, you will provide comprehensive nursing services to clients in southeast Toronto, particularly those experiencing homelessness and newcomers.

You'll work collaboratively to address their health needs through education, assessment, and care planning. Key Responsibilities:

- Deliver holistic nursing care to diverse clients
- Conduct assessments and implement health interventions
- Collaborate on client care plans within the team
- Lead immunization clinics and health education sessions
- Engage in community health initiatives and quality improvement Requirements:
- Current Registration with CNO as a Registered Nurse
- Baccalaureate degree in nursing or equivalent
- 3 to 5 years of nursing experience in community settings
- Basic Life Support certification
