In the Seniors Services and Long-Term Care Division, your mission will be to assist elderly clients in accessing essential services that enhance their quality of life. Key Responsibilities: • Assess functional status and eligibility of clients • Document client information and create comprehensive case files • Liaise with service providers for coordinated care • Manage a caseload with follow-ups and re-assessments • Respond to client needs and emergencies promptly

Requirements: • Degree or Diploma in Social Services or relevant experience • Considerable experience in community services • Strong interpersonal and organizational skills • Knowledge of community laws and support networks • Capable of working independently
