Provide exceptional in-home care as the Care Team Clinical Lead for VHA Home HealthCare in Toronto. This role focuses on client assessments, care plans, and personal support worker training in a temporary capacity. VHA Home HealthCare is recognized as one of Ontario's largest not-for-profit home care providers, seeking a Care Team Clinical Lead for a temporary contract.

As an experienced RN/RPN, you will play a pivotal role in creating and monitoring client care plans while ensuring effective support for personal support workers. You'll prioritize protected, client-centered care and engage in quality initiatives. Key Responsibilities:

- Conduct assessments to develop tailored care plans for clients
- Ensure documentation of concerns during home visits




- Provide clinical education related to nursing standards
- Collaborate closely with clients and families for effective care
- Offer after-hours clinical support as required Requirements:
- RN/RPN Degree or Diploma with current registration
- At least 2-3 years in community health preferred
- Experience with electronic health records required
- Valid driver’s license for home visits
