In this role, you will be a vital component of a multidisciplinary team dedicated to client and family-centered care. Responsibilities include monitoring clients, assisting them with accessing services, and providing emotional support. Key Responsibilities:

- Monitor clients and assess their living conditions
- Accompany clients to appointments and errands
- Link clients to community services and support
- Assist with accessing perks related to substance use
- Administer medications and handle emergency situations

Requirements:

- Must have Substance Use Certificate or similar credentials
- One year of relevant work experience
- Valid BC driver's license and vehicle
- Current CPR certification required
- Skills in crisis intervention and communication
