How will I be supporting the work of the Trust?

The Work and Health Coach is responsible for providing personalised support and guidance to individuals with disabilities or health barriers, helping them to enter, return to, or stay in employment by assessing clients' needs, developing tailored action plans, and coordinating with employers and other stakeholders to facilitate successful employment outcomes.

What does this involve?

- Carry out assessments and create personalised support plans to help participants move towards employment.
- Provide one-to-one coaching, motivation and ongoing tailored support to build confidence and work readiness.




- Signpost and connect participants to training, employment support services and suitable job opportunities.
- Build and maintain strong relationships with employers, promoting inclusive recruitment and reasonable adjustments.
- Work collaboratively with health, care and support services as part of a multi-disciplinary team.
- Monitor progress, maintain accurate records and evaluate the impact of support provided.
- Ensure all work is delivered in line with relevant legislation, policies and best practice.

Who are we looking for?

- Someone who is confident and knowledgeable in working with disability, with a robust understanding of its impact on individuals and those around them.
- Empathetic and person-centred, able to build trusting relationships and understand each client’s unique needs.
- Excellent communication skills, both written and verbal, with the ability to engage a wide range of audiences.
- Proactive and solutions-focused, able to identify barriers and support clients to overcome them.
- Highly organised, with strong time management skills and the ability to manage a caseload effectively.
- Flexible and adaptable, tailoring approaches to meet individual circumstances.
- Experienced in working collaboratively with clients, employers, healthcare providers and partner organisations.




- Good understanding of personalised care, coaching or motivational interviewing approaches, employment legislation, and welfare benefits.
- Experience supporting people with additional needs to access appropriate help, services and opportunities.
