Care & Surgery Centre, the Chronic Disease Nurse plays a pivotal role in developing tailored care plans for diabetes patients. This role emphasizes collaboration with healthcare teams and supporting self-management through personalized education in both group and individual settings. Key Responsibilities:
Develop and assess individualized care plans alongside healthcare providers
Facilitate education sessions for diabetes self-management
Maintain accurate records of patient care and teaching evaluations
Contribute to case reviews and educational resource development
Support current staff through orientation and mentoring Requirements:
Valid registration as a Registered Nurse with BCCNM
Two years’ relevant clinical experience in diabetes care
Certification in CPR and as a Diabetes Educator preferred
Excellent interpersonal and communication skills
