Durham Community Health Centre is seeking a Case Manager for the Welcoming Streets program. Focus on outreach and case management for marginalized communities in Oshawa. This position is a temporary full time role aimed at providing individualized support using the Housing First Model. Your work will involve collaboration with medical specialists to enhance health outcomes for individuals facing barriers such as homelessness and mental health issues across Durham Region. Key Responsibilities:
Provide outreach services in community locations
Assist clients with accessing housing and financial resources
Support program development and evaluation
Establish partnerships with local service providers
Document client interactions in relevant systems Requirements:
At least 2 years of experience in community outreach
Relevant post-secondary education in social services
Proficient in case assessment and intervention
Ability to work independently and within a team
Must possess a reliable vehicle Make a difference in the lives of vulnerable individuals as a Case Manager at DCHC.
