Health for All Family Health Team is an inter-professional primary care clinic where physicians, nurse practitioners, and allied health professionals work side by side to deliver comprehensive, patient-centered care. Our team has diverse clinical interests—palliative care, geriatrics, home visits, low-risk obstetrics, and global health—which means you'll have colleagues to collaborate with no matter where your passions lie.

We're also home to the Markham Family Medicine Teaching Unit, a partnership between Oak Valley Health–Markham Stouffville Hospital and the University of Toronto's Department of Family and Community Medicine. The Role:

You'll work closely alongside a dedicated group of physicians, contributing your clinical expertise across prevention, chronic disease management, and acute/episodic care—supporting a collaborative team model that values shared decision-making and mutual respect for each provider's skills and judgment.

What you'll do:
Conduct assessments, diagnoses, treatment, and health education in line with CNO standards for RN(EC)
Deliver holistic,



culturally responsive care that meets each patient where they are
Contribute to quality improvement initiatives and Family Health Team programs
Provide coverage for colleagues when needed
Engage in community outreach and health promotion activities

What We're Looking For:

Required
Current RN(EC) registration with the College of Nurses of Ontario (Primary Health Care stream)
Liability protection through the Canadian Nurses Protective Society

Preferred
Membership in RNAO and NPAO
Primary care or community-based NP experience
Experience working as a Nurse Practitioner in a Primary Care setting
Familiarity with EMRs (Telus Practice Solutions is a plus)
