In this role, you'll work alongside a multi-disciplinary team to create individualized care plans and ensure the implementation of services for adults with intellectual disabilities.

You'll advocate for client needs while managing a smaller caseload, enabling you to provide meaningful support and monitor service effectiveness. Key Responsibilities:
Identify client needs and develop tailored care plans
Coordinate and advocate for identified services
Track progress and assess service quality
Collaborate with the multidisciplinary team for optimal outcomes
Offer guidance for social work licensure support Requirements:
BA/BS in human services or similar discipline
Minimum 1 year experience in the IDD sector
Must have a reliable vehicle with appropriate insurance
Valid driver’s license with a clean record required
