Key Responsibilities:
• Perform initial and ongoing client assessments
• Develop personalized Client Care Plans based on needs
• Educate personal support workers on clinical practices
• Address client concerns through effective care strategies
• Support after-hours clinical visits when needed

Requirements:
• RN/RPN Degree with CNO registration
• Experience in community health, 2-3 years preferred
• Familiarity with electronic health record systems
• Ability to write and develop reports effectively
• Must hold a valid driver’s license
