SOAHAC is seeking an Indigenous Community Wellness & Outreach Coordinator to join our interdisciplinary team at the London site and support the Indigenous Mobile Health Clinic. You will coordinate activities, build relationships and help clients access culturally protected health services directly in communities.
You will work under the Integrated Care Manager with nurses, physicians and other team members, ensuring smooth care coordination, follow-up and continuity across clinics and partners.
