As a vital part of our Hospice Palliative Community Services, you will conduct social assessments, provide therapeutic counseling, and work alongside interdisciplinary teams. Your role will ensure robust advocacy for clients while enhancing service provision through collaboration with various community resources and organizations.

Key Responsibilities:
• Provide grief counseling and clinical support to families
• Conduct assessments using evidence-based practices
• Take part in the interdisciplinary care planning process




• Monitor ongoing psychosocial care and interventions
• Develop educational programs for clients and families

Requirements:
• Master’s degree in Social Work from an accredited institution
• Minimum two years' experience in a palliative setting
• Registered with the BC College of Social Workers
• Valid BC Driver's license with vehicle access
• Effective teamwork and communication skills are critical
