Vancouver Coastal Health is seeking a Mental Health and Substance Use Outreach Nurse in Richmond, BC. Serve the community by providing essential outreach services designed for diverse client needs.

You will work within the Assertive Community Treatment team, conducting thorough assessments and tailoring care plans to enhance client outcomes. Your responsibilities will include providing counseling, crisis intervention, and facilitating transitions between care facilities, ensuring a comprehensive support network for individuals facing severe mental health and substance use issues.

Key Responsibilities:
• Perform comprehensive mental health assessments
• Collaborate on effective client care plans
• Assist clients with accessing urgent and follow-up care




• Provide crisis intervention in acute situations
• Support clients in community engagement and services

Requirements:
• Active registration as RN or RPN with BCCNM
• Two years of related experience in mental health care
• Access to a personal vehicle for work-related travel
• Expertise in crisis management and client counseling
• Skilled in effective communication across health settings
