For over 35 years, The Safehaven Project for Community Living has been providing exceptional care to children with developmental disabilities and complex care needs.

Our mission is to provide high-quality, family-centered, and community-based residential and respite care for individuals with complex care needs through respite and residential programs at Toronto, Etobicoke, North York, Woodbridge and Aurora homes.

ROLE SUMMARY:

The Speech-Language Pathologist (SLP) provides assessment, consultation,



and intervention services to medically complex clients to support communication and swallowing needs. Using appropriate standardized and non-standardized tools, the SLP develops and implements individualized care plans in collaboration with clients, families, and the interdisciplinary team.

The SLP monitors progress, provides education to families and staff, makes referrals as appropriate, and ensures compliance with organizational, MCCSS, and professional documentation standards while promoting safe, client-centered care and continuous quality improvement.

RESPONSIBILITIES:

Provides consultation (including prevention), evaluation, assessment, intervention and counselling to clients and families around communication

Works in collaboration with a multi-disciplinary team to inform integrated care plans

Uses appropriate standardized tests and outcome measures in clinical practice

Makes appropriate referrals to other services, agencies, etc.

Incorporates goal setting in discussion with family/client

Adapts approach to changing needs of clients and families

Keeps family/client informed of progress and of any changes in plan

Adheres to Safehaven/MCCSS/professional documentation guidelines

Demonstrates familiarity with current literature, issues and trends of practice

Participates in regular quality improvement activities to further refine clinical practice,



equitable and efficient service for clients and families and to ensure the maintenance of a safe and healthy work environment

Shares new knowledge and best practices with colleagues, students and other professionals within the organization, professional body and community

Utilize the teaching/learning process to provide education, both formal and informal, to clients/families, colleagues, students, and other professionals within the organization and the community

Supports the maintenance of a protected and healthy work environment and advances a culture of client safety through work and daily practices

Other duties as assigned

QUALIFICATIONS:

Masters degree in Speech Language Pathology

Current registration with the College of Audiologists and Speech Language Pathologists of Ontario (CASLPO)

Previous pediatric clinical experience preferred

Experience with providing assessment, consultation and therapy for medically complex and/or inpatient rehab preferred.

Demonstrated ability to communicate effectively with clients, families and members of the interdisciplinary care team

Demonstrates flexibility and co-operation in dealing with change

Demonstrated ability to work independently and in an interdisciplinary team
