We are seeking a proactive Social Worker to join Safehaven, where you will offer case management, community outreach, and advocacy for individuals with developmental disabilities. This position includes clinical assessments, participation in case conferences, and educational outreach to community members. Work across various locations in the GTA to meet operational needs and improve lives through dedicated care and engagement.

Key Responsibilities:
• Deliver clinical services to families and individuals
• Coordinate case management with community resources




• Engage in program planning and evaluation
• Facilitate intake consultations for current clients
• Present educational sessions to promote awareness

Requirements:
• Bachelor's in Social Work, Master's degree preferred
• Minimum of three years of clinical experience
• Knowledge of community resources and disability services
• Proficient in communication and technology
• Valid driver’s license and access to a vehicle
