As a Care Partner, you will travel across Eastern and Southern Metropolitan Melbourne to provide personalized services. Your expertise will help develop care plans and assess client needs effectively. Building relationships with clients is crucial for enhancing their quality of life and ensuring their needs are met with dignity and respect.
Key Responsibilities:
• Deliver individualised care and support services
• Conduct assessments and maintain case notes
• Collaborate with multidisciplinary teams for client wellbeing
• Manage and coordinate a client service portfolio
• Support clients in making informed choices regarding their care
Requirements:
• Diploma or higher in relevant fields
• Background in aged care or client services
• Excellent organizational abilities and autonomy
• Understanding of aged care standards and reforms
• Current Victorian Driver's Licence required
