Provide exceptional in-home care as the Care Team Clinical Lead for VHA Home HealthCare in Toronto. This role focuses on client assessments, care plans, and personal support worker training in a temporary capacity.

VHA Home HealthCare is recognized as one of Ontario's largest not-for-profit home care providers, seeking a Care Team Clinical Lead for a temporary contract. As an experienced RN/RPN, you will play a pivotal role in creating and monitoring client care plans while ensuring effective support for personal support workers. You'll prioritize protected, client-centered care and engage in quality initiatives.

Key Responsibilities:
• Conduct assessments to develop tailored care plans for clients
• Ensure documentation of concerns during home visits




• Provide clinical education related to nursing standards
• Collaborate closely with clients and families for effective care
• Offer after-hours clinical support as required

Requirements:
• RN/RPN Degree or Diploma with current registration
• At least 2-3 years in community health preferred
• Experience with electronic health records required
• Valid driver’s license for home visits
• Familiarity with health legislation and standards needed
