Join Grey County's initiative to ensure housing stability as a Housing First Case Coordinator. Provide critical support and resources for individuals moving into new homes after experiencing homelessness.

In this 8-month contract, you will be responsible for managing a defined caseload of newly housed clients. Your role includes facilitating access to housing resources, educating about tenant responsibilities, and addressing any emerging needs to ensure a smooth transition. With a strong focus on relationship-building with landlords, you will help to create a supportive community environment for tenants.

Key Responsibilities:
• Deliver case management for up to 12 households
• Assist with move-in preparations and utility setups
• Connect clients to local services and financial supports




• Conduct regular check-ins to assess client needs
• Educate on tenant rights to ensure long-term success

Requirements:
• At least 3 years’ experience in social services
• Relevant education in social or behavioral sciences
• Knowledge of Housing First and related community services
• Solid interpersonal and analytical skills
• Valid driver’s license required with access to transportation
