Become a vital part of the community by joining March of Dimes Canada as a Navigator of Community Services in Whitby, ON. This full-time role is centered on consumer assessments and collaboration with local agencies.

Operating from 1615 Dundas Street East, Suite 305, this position runs Monday to Friday from 9 a.m. to 4:30 p.m., providing 35 hours of dedicated service weekly. As a Navigator, you will conduct interRAI CHA assessments, renew service agreements, and manage intake processes for consumers while supporting their access to community resources.

Key Responsibilities:
• Complete interRAI CHA assessments for recent consumers
• Renew annual agreements for existing clients
• Oversee intake and referrals effectively




• Liaise with community and agency stakeholders
• Manage waitlists and maintain organized files

Requirements:
• Post-secondary education in health/human services
• Experience with interRAI CHA assessments preferred
• Must have current CPR/First Aid certification
• Excellent communication and organizational skills
• Valid Driver's License with reliable transport necessary
