POSITION: Nurse Practitioner (NP)

STATUS: RN (EC) – Two (2) Full-Time Position

SCHEDULE 5 days a week, may include occasional weekend

DEPARTMENT Management On-site and In-person: No remote/hybrid

LOCATIONS

1. Wellesley Central Place. 150-bed Long Term Care Home located Downtown Toronto
2. Sherbourne Place. 90-bed Long Term Care Home located Downtown Toronto

Reporting Structure The NP reports to the Executive Director for day-to-day operations. Collaborates with the Director of Nursing Services and Medical Director (MD) to help support and align with our resident care needs.

Responsibilities:

- Conduct comprehensive health assessments of residents, including physical examinations, medical history reviews, and medication reconciliation.
- Develop and implement individualized care plans based on residents' needs, goals, and preferences.
- Provide direct clinical care, including ordering and interpreting diagnostic tests, prescribing medications, and performing minor procedures within the Nurse Practitioner scope of practice.
- Collaborate with physicians, registered nurses, allied health professionals, life enrichment professionals and other team members to optimize resident outcomes and quality of life.
- Educate residents, families, and caregivers on health promotion, disease prevention, and self-management strategies.
- Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives to enhance and optimize resident care and safety.
- Maintain accurate and up-to-date documentation of assessments, interventions, and outcomes in eMAR.
- Adhere to professional standards, regulatory requirements, and best practices in nursing and healthcare delivery.
- Support the Emotion-based care approach and the 2SLGBTQ+ initiative.
- Mentor Students
- Other Activities – Other activities may be assigned in collaboration with the inter- professional team

Skills, Knowledge Requirements

General

- Leadership, critical thinking, organizational and communication skills are required




- Expertise in developing and maintaining excellent working relationships with all members of the health care team and partner organizations
- Ability to work in a collaborative practice model
- Advance health assessment/clinical skills with a high degree of accuracy and attention to detail
- Ability to prioritize, manage time effectively and be flexible in a fast paced, energetic work environment
- Must possess a dependable, consistent and reliable work record including attendance at work
- Ability to maintain confidentiality and impartiality

Computer Skills

- Strong knowledge of Electronic documentation of health records ie CMS, PCC
- Knowledge of and working experience with computer systems and applications

Language Skills: Proficient written and verbal English skills required. Any second language is considered an asset.

Education and Specific Job Requirements

Required

- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class Primary Health Care or Adult and entitle to practice with no restrictions/limitations
- CPR Designation designed for HealthCare Professionals and Standard First Aid
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management and treatment of older persons with delirium, depression, cognitive impairment, confusion and behavioural disturbance
- Vulnerable Sector Check not less than 6 months old

Considered an Asset

- Certification in Geriatric Nursing from the Canadian Nursing Association or equivalent experience
- Certifications obtained or in progress that include gerontology, mental health, behavioral support and palliative care.
- Working Knowledge of Fixing Long Term Care Act, 2021, Food and Drugs Act (Canada) 1985, and Narcotic Control Act (Canada) 1994.
- Current membership: Nurse Practitioners' Association of Ontario (NPAO) is preferred.
- Teaching experience within the Nurse Practitioner scope

Relevant Experience

- Two years’ clinical experience as a Nurse Practitioner - preferred
- New graduates of recognized Nurse Practitioner program are welcome to apply
