Overview

The Stratford Family Health Team (SFHT) is seeking a dedicated and cooperative Nurse Practitioner (NP) to join our interdisciplinary care team. The success full candidate will be employed by the Stratford Family Health Team and assigned to work within the Mitchell Family Health Organization (MFHO) clinic in Mitchell, Ontario, providing primary care services in support of MFHO physicians and their patients.

Working closely with primary care physicians, clinic staff, and team members, the NP will provide comprehensive, patient-centered care to patients rostered to the MFHO physicians. The successful candidate will assess, diagnose, treat, and manage a wide range of acute and chronic health conditions, promote health and disease prevention, and contribute to improved patient outcomes across the lifespan.

Responsibilities

- Provide comprehensive primary care to patients rostered to MFHO physicians
- ordering and interpreting tests, prescribing medications, and developing and implementing individual treatment plans
- Collaborate with physicians to support access, continuity, and quality of care for the practice population
- Diagnose and manage acute and chronic medical conditions within the NP scope of practice
- Maintain accurate, timely, and comprehensive electronic medical records

Qualifications

Required

- Current registration in the Extended Class RN(EC) with the College of Nurses of Ontario in good standing
- Completion of a recognized Primary Health Care Nurse Practitioner program
- Current professional liability protection as required by the College of Nurses of Ontario




- Authorized to practice as a Nurse Practitioner in Ontario
- Demonstrated knowledge of primary care and family medicine principles
- Strong clinical assessment, diagnostic, and decision-making skills
- Ability to work independently while collaborating effectively within a team environment
- Proficiency with electronic medical records (EMRs) and computer-based documentation

Preferred

- Previous experience working in a primary care clinic, family health team or organization, or community health centre
- Experience providing comprehensive primary care across the lifespan
- Experience managing chronic diseases such as diabetes, hypertension, COPD, asthma, and mental health conditions
- Experience providing care across all age groups
- Previous experience functioning as a Most Responsible Provider (MRP)

Compensation: $127,987

Hours/week: 37.5 hours

Paid sick leave: 5 days annually

Paid personal leave: 5 days annually

Vacation: 4 weeks to start
