This is a current opening with our client, a multi-service centre providing primary health care in Rexdale, ON. Our client is looking for a Bilingual Social Worker who will work largely with Black youth/clients and their families, as appropriate, who are experiencing crisis and/or are at risk of experiencing a crisis, including those who are dealing with mental illness and/or co-morbid addiction.

This position delivers care within our client’s model, recognizing the lived experience of Black youth who have faced anti-Black racism and oppression as it relates to the many aspects of their lives.

This is a full time and permanent role. This position requires a 4-day work from Rexdale office and 1 day work from home (virtual). Salary: $65,327.00. - $78,708.00. Full benefits after 3 months, 4 weeks vacation.

Responsibilities

The Social Worker assesses and supports community members who are at risk of experiencing a crisis and/or are currently experiencing a crisis, with the goal of providing culturally appropriate support, including system navigation and timely referral to appropriate services, as well as short-term crisis intervention work such as counselling and case management.

The Social Worker applies this crisis assessment and intervention function specifically to Black youth (ages 14–29) and their families, delivering care that is holistic, healing-centred, Afrocentric, and culturally responsive. This includes providing consultation and referrals to other providers serving this population as necessary, and supporting clients across the continuum of prevention, early intervention, harm reduction, and recovery.

- Engages in short-term assessment, referral, and advocacy activities.
- Acts as a resource to advocate for individuals, groups, and the community.
- Provides risk assessment, personal and practical support, information, referral and advocacy with individuals.
- Assesses client status by gaining an understanding of social determinants of health.
- Acts as a client advocate on individual and systemic issues.
- Provides crisis intervention services both over the telephone and in person.
- Contributes to chart documentation, participates in chart reviews and case conferences, reflective practice meetings/discussions and referrals.
- Participates in the development, implementation, monitoring and evaluation of treatment, education,



counselling and health promotion programs and services for individuals, families and the community.
- Acts as a consultant on resources for other service providers in the Centre and the community.
- Liaises and collaborates with staff, teams and other community service providers as appropriate to provide service and make referrals.
- Assists clients and the community in building support networks.
- Ensures accountability by providing statistics and reports of activities, as required, and keeping client records, as needed, within the safeguard of confidentiality
- Provide individual and/or group therapy using CBT, DBT, and other evidence-based approaches.
- Develop group therapy programs and facilitate group therapy sessions.

Staff Team Maintenance

- Discuss and resolve any problem/issue/concerns with the supervisor and volunteers
- Provide constructive criticism and positive feedback to volunteers

Community Development/Outreach

- Assist program participants and community members in identifying issues and concerns and developing strategies to address them.
- Exchange information and resources with the staff team and other community organizations.

Occupational Health & Safety

- As a member of staff at the Community Health Centre, you have direct responsibility for Health and Safety as an essential part of your job and must work in compliance with the Occupational Health and Safety Act & Regulations.

You are required to:

- Wear/use protective equipment (where required)
- Report hazardous situations to your supervisor
- Report all incidents/accidents to the supervisor immediately
- Use/operate equipment in a safe manner
- Engage in all health and safety programs, including emergency procedures, fire drills, and evacuations, following company policies and regulations
- Refrain from pranks or contests

Qualifications

· University degree in social work, counselling, psychology or related field.





- Current Registration with an appropriate regulatory college, e.g. OCSWSSW, CPO, CRPO.
- Minimum three years of experience in intake and assessment within a mental health or clinical setting.
- Minimum three years of counselling/therapy experience, preferably in a community-based health setting with youth and their families.
- Must have excellent language skills in both English and French
- Proven skills in psychosocial assessment, counselling/therapy, crisis intervention, case management, and referrals.
- Demonstrated skills and experience in working with racialized communities.
- Demonstrated commitment to and lived experience of racialized communities.
- Knowledge of community resources.
- A deep, fundamental, comprehensive knowledge of the unique challenges racialized communities face across the social determinants of health.
- A commitment to an interdisciplinary team approach to the coordination of health and social services.
- Proficiency in current assessment, counselling/therapy, and psychosocial counselling techniques for working with individuals, groups, and communities.
- Demonstrated skills and experience in assessing and treating substance abuse and/or mental health, including individual and group counselling.
- Strong communication skills (verbal and written) combined with computer literacy are required.
- Ability to negotiate, communicate and maintain a supportive, trusting relationship with those being served.
- Experience in program development, implementation, and evaluation.
- Knowledge of community-based health services.
- Strong and demonstrated ability to form relationships with individuals while maintaining healthy boundaries.
- Caseload management skills, including developing action plans for clients, monitoring client progress and maintaining records.
- Working knowledge of the Ontario Mental Health Act.
- Knowledge of the Personal Health Information Protection Act, Consent of Information / Records of Information and Medical Records Management.
- Knowledge of the Rexdale community.

· Must be able to provide a vulnerable sector security clearance.

Pay: $65,327.00-$78,708.00 per year
