Position overview
- The Outreach and Goals Support Worker support children and youth with their behavioral needs, individual goals, and community engagement.
- Working closely with Intake Outreach Coordinator, the Worker reviews incident reports and support plans, identifies gaps in support planning, and schedules and implements individualized outreach supports. The position also assists with emergency intakes, discharges, and related administrative duties.
- During designated hours in each program, the Worker provides direct support, observes needs, and helps caregivers apply consistent strategies.
- The role connects practical support with clear documentation and timely updates to Intake Outreach Coordinator and management through the agency’s designated on-call email.
Goal review and progress monitoring
- Review each child or youth’s goals with the youth, caregivers, and care team to ensure they reflect current needs, strengths, and priorities.
- Break goals into practical, achievable steps and support their implementation during program visits and outreach activities.
- Track progress in emotional regulation, relationships, education, daily living skills, cultural connection, and community participation.
- Identify barriers, record achievements, and recommend adjustments to the Intake Outreach Coordinator and the care team.
Behaviour support and support planning
- Review incident reports (IRs) to identify patterns, triggers, unmet needs, and strategies that have been helpful.
- Review behaviour support and safety plans alongside direct observations, youth feedback, and caregiver input to identify gaps in planning or implementation.
- Assist with updating support plans with Intake Outreach Coordinator, supervisors, and relevant clinical supports; implement strategies once approved.
- Support caregivers with co-regulation, de-escalation, predictable routines, positive reinforcement, and restorative follow-up. Document outcomes and promptly escalate significant concerns.
Outreach planning and implementation
- Develop, schedule,
and implement outreach activities aligned with each youth’s goals, interests, cultural connections, and approved support plan.
- Coordinate appointments, community resources, staffing needs, and transportation arrangements with Intake Outreach Coordinator and the program team.
- Provide direct support for life skills practice, school engagement, recreation, mentorship, and meaningful community participation.
- Follow approved supervision and safety requirements; document participation, progress, and barriers, and recommend adjustments.
Emergency intakes and discharges
- Assist Intake Outreach Coordinator with emergency intake preparation, information gathering, documentation, and communication with the receiving program.
- Help identify immediate needs, existing support strategies, and outstanding information; support a welcoming orientation and initial adjustment.
- Assist with initial support planning and follow-up tasks in collaboration with the program team.
- Support planned and emergency discharges by coordinating belongings, transition documentation, authorized information sharing, and handovers as directed.
Administrative duties and communication
- Use the agency’s designated on-call email to send timely updates to Intake Outreach Coordinator and management on youth goals, behavioral concerns, outreach activities, and gaps in support planning.
- Provide updates on IR reviews, support plan follow-ups, emergency intakes, and discharges; clearly identify completed actions, outstanding tasks, and decisions required.
- Maintain accurate records, prepare progress summaries, schedule reviews, and follow up on assigned administrative tasks.
- Share information only with authorized recipients and maintain confidentiality. Escalate urgent safety concerns immediately through agency on-call procedures, alongside written documentation.
Scheduling and accountability
- Work designated hours in each program according to youth needs and priorities set with Intake Outreach Coordinator and management. Maintain flexibility for changing outreach plans, intakes, and discharges.
- Coordinate scheduling changes with Intake Outreach Coordinator management and plan all direct support, outreach, administrative work, and meetings within the maximum of 20 hours per week.
- Record all hours worked accurately and submit timesheets according to agency procedures. Raise competing priorities promptly so work can be reassigned or rescheduled.
Qualifications and skills
Relevant education or experience in child and youth care, social services, community outreach, or behavioral support is preferred. The Worker should demonstrate solid observation, documentation, communication, and organizational skills; confidence using email and electronic records; and the ability to build respectful relationships and respond calmly to changing needs.
Practice expectations
Provide trauma-informed, culturally responsive, and restorative support that honors each youth’s voice, identity, family relationships, and cultural connections. Follow agency policies, approved plans, professional boundaries, and delegated responsibilities. Seek direction for decisions requiring management or clinical approval and complete training required for assigned duties.
Pay: $19.50 per hour
Expected hours: No more than 20.0 per week
Education:
- Secondary School (preferred)
