Case Management – Caseworker

Reports To

Settlement Services Coordinator and Executive Director

Program Summary In support of the mission of Adult Language and Learning the Case Management Program provides intensive, client-centered support to newcomers facing multiple and complex barriers to settlement. Guided by the Immigration, Refugees and Citizenship Canada (IRCC) Case Management Framework, the program delivers coordinated, structured, and goal-oriented services that support clients in achieving sustainable settlement and integration outcomes.

Job Purpose The Case Management Case Worker provides individualized, intensive support to high-needs newcomers through comprehensive assessment, case planning, coordinated service delivery, and ongoing monitoring.

The role focuses on clients experiencing complex barriers such as mental health challenges, housing instability, family violence, social isolation, and system navigation difficulties. The Case Worker develops structured service plans, coordinates multi-sector supports, and monitors progress toward defined settlement goals.

Job Duties

This Job Profile describes the general nature and level of work being performed by employees assigned to this position. Employees may perform other duties and tasks as required to meet the needs of the operation. The responsibilities listed within the job profile are only guidelines.

1. Intake, Assessment & Eligibility

- Conduct comprehensive intake and eligibility screening aligned with IRCC CMF criteria
- Complete detailed needs and risk assessments (including safety, health, and social determinants)
- Identify clients requiring short-term vs. intensive case management support
- Prioritize high-needs and complex cases

2. Case Planning & Goal Setting

- Develop individualized Case Management Plans with clear, measurable goals
- Collaborate with clients to identify priorities, strengths, and barriers
- Establish timelines, action steps, and expected outcomes
- Ensure plans are client-centered, culturally appropriate, and trauma-informed

3. Service Coordination & System Navigation

- Coordinate services across sectors (healthcare, housing, legal, education, employment, social services)
- Facilitate warm referrals and actively follow up with service providers
- Advocate on behalf of clients to reduce systemic barriers
- Support clients in navigating complex systems (e.g., healthcare, income supports, immigration processes)

4. Ongoing Case Management & Monitoring

- Maintain regular contact with clients (in-person, virtual, phone)
- Monitor progress against case plan goals and adjust as needed
- Conduct risk reassessments and respond to emerging needs
- Provide crisis intervention and safety planning when required
- Client accompaniment and referrals




- Transition clients out of case management when goals are achieved

5. Documentation & Reporting

- Maintain detailed, accurate, and timely case notes in accordance with IRCC requirements (e.g., iCARE/OCMS)
- Track outcomes, progress, and service interventions
- Ensure compliance with privacy, confidentiality, and data management standards
- Contribute to program evaluation and reporting

6. Collaboration & Case Conferencing

- Participate in internal case reviews and multidisciplinary case conferences
- Liaise with external service providers to coordinate care
- Build partnerships with community agencies and stakeholders
- Share knowledge and contribute to team learning

7. Client Support & Advocacy

- Provide direct support to clients experiencing crisis or vulnerability
- Advocate for equitable access to services and supports
- Promote client empowerment, independence, and self-advocacy
- Deliver services in a culturally safe, inclusive, and trauma-informed manner

Job Requirements · Diploma/Degree in Social Work, Social Service Work, Psychology, or related field

· Minimum 2–3 years experience working with vulnerable populations

· Experience in case management or intensive client support (strongly preferred)

· Experience / knowledge of newcomer settlement challenges and community resources

· Understanding of trauma-informed and client-centered approaches

· Experience with data systems (e.g., iCARE/OCMS) an asset

· Ability to work individually as well as part of a team

· Ability to prioritize and manage conflicting demands while effectively and accurately managing client caseloads, client data, and document client progress.

· Proficient in Microsoft and Adobe Reader (Outlook, Teams, Word, Excel, PDF, etc.

· Proficiency in English

· Fluency in English required; additional languages an asset

· Must provide a current Criminal Record Check as per the ALL’s policy

· Valid driver’s license and access to a vehicle for offsite work required

Work Conditions





· High emotional demand due to complex client situations

· Frequent exposure to crisis situations and vulnerable populations

· Requirement for flexibility and responsiveness

· Office environment with moderate exposure to public speaking and training facilitation. Some driving between sites and some off-site work may be necessary.

· Moderate physical requirements to stand and facilitate sessions, bending and lifting to set up programs, manual dexterity, and light to normal physical activities such as keyboarding, filing, climbing stairs, carrying, and lifting low-weight items.

· Continuously upgrades and maintains knowledge of changing Immigration Legislation, trends, and developments in the ever-changing cultural environment

· Frequent cognitive requirements with periods of sustained concentration, client interaction, presentation, speaking to a group of individuals, emotional situations, decision-making and recommendations, and paperwork.

· Interaction with employees, management, employers, clients and the public at large

Details:

· Full- time

· This role mostly works regular hours 8:30 - 4:30 pm, and may be on call depending on client needs requiring oversite.

· Available to work flexible hours, including evenings and weekends
