Hours of Work: Monday to Friday, 8:30 am-4:30 pm

Location: We currently have three(3) 14-month contract positions available within York Region including Maple/Vaughan, North (Newmarket, Keswick, Pefferlaw) and West (Woodbridge to Schomberg)

*Please note that frequent travel is required within the service area as well as to the head office in Aurora.

POSITION SUMMARY: The Client Care Supervisor is responsible for the overall management and functioning of Personal and Home Support Services.

Responsibilities

- Communicates with client, family, case managers and staff regarding scheduling, care plan goals and service delivery, as needed.
- The Client Care Supervisor will be aware of and adhere to all policy and procedures as set forth in the Policy and Procedure Manual of RNS Health Care Services.
- Participate in the on-call rotation
- Monitoring, training and assisting Personal Support Workers in the delivery of Personal Support and Homemaking Services to Clients.
- Completing initial and ongoing assessments of recent clients to ensure client needs are met and maintained by RNS Health Care Services.
- The Client Care Supervisor will also promote the services of RNS Health Care Services to the community.

Qualifications

- Completion of Bachelor`s degree in Registered Nursing or Registered Practical Nurse
- In good standing and registered with CNO
- 2 years of experience managing in a multidisciplinary, culturally diverse home health care environment




- Knowledge and experience with CCAC
- Own transportation and valid driver’s licence
- Must be able to travel frequently within the service area (Thornhill) and travel to our Newmarket office

- Computer knowledge of MS Office, Excel, MS Word, and MS PowerPoint
- Strong team leadership, relationship-building and supervisory skills
- Commitment to providing the highest level of customer service and client care
- Demonstrated organization and time management, facilitation, negotiation and problem-solving skills
- Ability to plan, prioritize and organize
