Six Nations of the Grand River invites applications for the First Link Care Navigator contract role. Based out of Six Nations, you will coordinate supports for persons living with dementia and their care partners, serving as the primary contact after diagnosis and driving care planning across health and community services. You will lead information sharing, problem solving, and liaison with physicians, hospitals, and community providers to enhance home-based and community care outcomes.
