Rexdale Community Health Centre (RCHC) is a non-profit, multi-service centre providing primary health care, settlement and community development activities for women, families, youth, adults, and seniors of diverse cultures. Job Summary:

RCHC is currently seeking a full-time Nurse Practitioner for a 1-year contract (Maternity Leave Coverage) who will promote health and prevent illness through primary care, community health services and health education. RCHC's service delivery incorporates principles of advanced access, using group visits and self-management models. We are seeking an individual committed to these principles. The successful candidate will be a member of the Centre’s clinical team.

The Nurse

Practitioner will also assist the "Wheels to Wellness" bus going through the community and meeting people where they are.

The position reports to the Director, Primary Health Care Services.

Responsibilities:

· Provide primary care to individuals and their families, in office and in the community, taking into account risk factors (environmental, socio-economic, psychological and medical) in a manner which can be understood by the client and, when applicable, their families

· To provide health care, treatment plans, health education and support for clients

· To participate in the development, implementation and evaluation of treatment, education, counselling and health promotion services for individuals, families and the community

· To order/interpret results of screening, diagnostic lab tests, X-rays and ultrasounds

· Provide clinical care to pre- and post-natal pregnant women and their newborns, including breastfeeding support programs.

· To determine the need for and implement health promotion, primary and secondary prevention strategies for individuals, families and communities





· Explore creative service delivery approaches

· Advanced knowledge and proficiency in current, evidence-based methods and practices of primary care delivery with an emphasis on health promotion, risk reduction and injury prevention

· To support and counsel clients in managing their chronic disease

· Familiarity with client-centred and self-management approaches to chronic disease prevention and management

· Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model

· Maintain and develop competence through appropriate continuing education methods

Qualifications

· Baccalaureate nursing degree with Extended Class Certificate of Competence from CNO in primary health care

· Registration with the College of Nurses of Ontario with professional liability insurance

· Affiliation with the Nurse Practitioners’ Association of Ontario (NPAO)

· Experience working as a Registered Nurse Practitioner in a community setting is an asset

· Knowledge of the Personal Health Information Protection Act, Consent of Information / Records of Information and Medical Records Management

· Familiarity with electronic medical records, e.g. PSS is an asset

· Proficient with computer software including MS Office, Internet and email

· Ability to work effectively both independently and as a member of an interdisciplinary team

· Demonstrated ability to work well with women, youth, marginalized populations, and diverse ethnoracial and cultural groups





· Demonstrated experience in all aspects of program development and implementation

· Awareness of and sensitivity to the health issues of a diverse community

· Sound knowledge of health promotion and disease prevention is an asset

· Excellent verbal and written communication skills

· Demonstrated ability to multitask and be flexible and organized

· Must be able to provide criminal record check clearance

· Ability and willingness to travel

· Knowledge of the Rexdale community

· Ability to speak a second language (e.g. French, Spanish, and Arabic) will be an asset.

· Able to work Monday to Friday from 9 am to 5 pm with some weekends and evenings as needed.

Salary: $108,091 to $130,231(depending on experience)

Hours per week: 35 hours per week

Duration: 1 year (Maternity Leave Coverage)
