Position Summary The Occupational Therapist is responsible for assessing patients' physical, cognitive, emotional and functional abilities and developing individualized treatment plans to help them achieve greater independence and participate safely in daily activities.

The successful candidate will provide evidence-informed, patient-centred occupational therapy services across a range of healthcare and community settings. The Occupational Therapist will work collaboratively with physicians, nurses, physiotherapists, social workers and other healthcare professionals to support patient recovery, rehabilitation and quality of life.

Key Responsibilities

- Conduct comprehensive occupational therapy assessments of patients' functional, physical, cognitive and psychosocial abilities.
- Identify barriers affecting patients' ability to participate in activities of daily living, work, education and community life.
- Develop individualized treatment and rehabilitation plans based on assessment findings and patient goals.
- Provide occupational therapy interventions to improve patients' functional abilities, independence and quality of life.
- Assist patients with activities of daily living, including personal care, dressing, bathing, feeding and household activities where applicable.
- Provide therapeutic activities and exercises to improve strength, coordination, mobility, fine motor skills and functional performance.
- Assess cognitive, perceptual and psychosocial functioning and provide appropriate interventions.
- Recommend and facilitate the use of adaptive equipment, assistive devices and environmental modifications.
- Assess patients' home,



workplace or community environments where required and recommend appropriate modifications to promote safety and independence.
- Provide education and training to patients, families, caregivers and other healthcare professionals.
- Develop home-based programs and strategies to support continued rehabilitation and independence.
- Support patients recovering from illness, injury, surgery, neurological conditions, developmental conditions or other disabilities.
- Provide rehabilitation services for individuals with physical, cognitive, developmental or mental health-related functional challenges.
- Monitor patient progress and modify treatment plans based on clinical outcomes and changing needs.
- Document assessments, treatment interventions, patient progress and discharge recommendations accurately and promptly.
- Communicate patient progress and treatment outcomes to physicians and other members of the healthcare team.
- Participate in interdisciplinary care planning, case conferences and discharge planning.
- Collaborate with physiotherapists, nurses, physicians, speech-language pathologists, social workers and other allied health professionals.
- Promote fall prevention, patient safety, independence and participation in meaningful activities.




- Assist with return-to-work, school or community reintegration programs where applicable.
- Maintain patient confidentiality, privacy, dignity and skilled boundaries.
- Follow infection prevention and control procedures, workplace health and safety requirements and organizational policies.
- Participate in continuing education, professional development and quality improvement initiatives.
- Perform other duties within the scope of occupational therapy practice as assigned.

Qualifications & Requirements

- Completion of an accredited Occupational Therapy education program.
- Eligibility for or current registration with the Ontario occupational therapy regulatory body.
- Candidates must meet the registration and licensing requirements of Ontario, Canada.
- Previous clinical experience in occupational therapy, rehabilitation, acute care, community care, mental health, long-term care or a related setting is an asset.
- Current CPR/BLS certification is required.
- Strong clinical assessment, reasoning and treatment-planning skills.
- Excellent communication, interpersonal and patient-education skills.
- Strong documentation and organizational abilities.
- Ability to work independently and collaboratively within an interdisciplinary healthcare team.
- Commitment to evidence-informed and patient-centred care.

Professional & Regulatory Requirements The successful candidate must be legally eligible to practise as an Occupational Therapist in Ontario, Canada and maintain registration in good standing with the relevant regulatory body.
